The United States Premier Hockey League was founded in November of 2012 with several organizations that had split away from the Eastern Junior Hockey League..  The league was originally formed with three divisions (Premier, U18 and U16).  On June 6, 2013 the league announced it had merged with from teams in the following leagues:  Empire Junior Hockey League and the newly formed Eastern Elite Hockey League, which was a merger of the remained of the teams from the Eastern Junior Hockey League and the EJHL South ,   The expanded league would operate five leagues including "Premier" (Tier-III). "Elite"(Tier-III), "Empire" (Tier-III), "18U", "16U". Until the end of the 2016-17 season all leagues were sanctioned by USA Hockey.  The five leagues started play in the fall of 2013.  The league is based in Marlborough, Massachusetts.  Each of the three Tier III level leagues had their champion qualified for the 2014 USA Hockey Tier III Junior National Championship

For the organizations second season (2014-15) the league added a U-16D (Under-16 development) league The Premier League did not participate in the 2015 USA Hockey Tier III Junior National Championship

The league announced the formation of a Mid-West Division on December 18, 2014 scheduled to commence play for the 2015-16 season.  The original membership of the division was the entire Central Division of the Minnesota Junior Hockey League and all but five of the Minnesota Division teams.  On March 10, 2014 the whole membership of the Midwest Junior Hockey League was added to the newly formed division and at by late April the whole membership of the Minnesota Junior Hockey League (including two of three dormant franchises) except for the Rochester Ice Hawks and the dormant Twin Cities Northern Lights had joined the new division.  The league had also added the Kalkaska Rhinos formerly of the Canadian International Hockey League as well.  The Minnesota Junior Hockey League would  announce that it was disbanding for the 2015-16 season on April 29th. On January 14, 2016 the USPHL announced the formation of youth divisions at the major and minor levels to begin for the 2016-17 season. The new divisions would involve 14 and under (Bantam), 12 and under (Peewees), and 10 and under (Squirts).

The league announced on April 28, 2016 that the USPHL Midwest Division was being broken up and being reorganized as part of the USPHL Elite Division and USP3HL with 9 teams going to the forming the Midwest Division of the USPHL Elite  and 9 teams going to the forming the Midwest Division of the USP3HL.  Included in these 18 teams will be the return of the Ironwood Fighting Yoopers who went dormant prior to the 2015-16 season due to a lack of players.

The league announced on May 6, 2016 that players 18 and over would be able to play wearing the half-shield visor starting with the 2016-17 season.

On May 20, 2016; the league announced that the four remaining members of the Northern Pacific Hockey League would form the USPHL's new USP3 Pacific Division. As part of the release a statement from the now former NPHL commissioner Garry Swain indicated that the four teams unanimously decided to join the USPHL citing the league's track record of player development since its founding.

2017-18 changes[]

On October 6, 2016 the league announced its' intention to offer a tuition free-division (Tier-II) of junior hockey in 2017-18. The league stated that it had submitted a formal application to USA Hockey to be a certified Tier 2 league pending the approval of USA Hockey. The release mentioned eleven members to be in the new league (Boston Bandits, Boston Jr. Bruins, Connecticut Jr Rangers, Islanders Hockey Club, Jersey Hitmen, New Jersey Rockets, Northern Cyclones, P.A.L. Jr. Islanders, Rochester Monarchs, South Shore Kings, and the Syracuse Stars. USA Hockey denied the application for Tier-II status for the league at a meeting of the junior hockey council in January 2017; but the league has gone forward with plans to play as a Tier-II league. Apparently, the main issue with the proposal was the method in which the league was being funded; the United States Hockey League and North American Hockey League are based on ticket revenue (as the teams in these leagues tend to play in larger arenas which are usually owned by third parties such as local/county governments) and advertising, while the USPHL planned on funding the new league with a tax (or higher playing fee) on players in lower tiers to offset the cost as most of the team owners in the USPHL organization are the arena owners and run programs from junior down to youth levels and run their operations mostly based on player fees instead of ticket sales in mostly smaller arenas.

On December 22, 2016 the USPHL announced that the Boston Bandits, Connecticut Nighthawks, Hartford Jr. Wolfpack, New Hampshire Jr. Monarchs, New Jersey Rockets, and Northern Cyclones organizations are leaving the Eastern Hockey League and joining the United States Premier Hockey League for the 2017-18 season. This move would bring six premier level and five elite junior teams. The 16U, 18U and youth programs of these organizations would also be involved in this move. The Boston Bandits, New Jersey Rockets, and Northern Cyclones would be also be part of the National Collegiate Development Conference when it commences play for the 2017-18 season. In March 2017, the league announced that the USP3HL was being retired with the Premier and Elite Divisions remaining. The league also announced that the Jersey Shore Whalers and Florida Jr. Blades would be added to the Premier Division along with the Skipjacks Hockey Club. The Jr. Blades were also moving their USP3HL team to the Elite division. With the reorganization of the leagues many of the teams would move up from the Elite to Premier Division and from the USP3HL to the Elite Division for 2017-18. As part of fallout from the rejection of Tier-II status for the National Collegiate Development Conference; the whole United States Premier Hockey League withdrew from USA Hockey sanctioning and will be unsanctioned for the 2017-18 season. Previous speculation indicated that the USPHL may affiliate with the Amateur Athletic Union's United Hockey Union for 2017-18. A possible return to USA Hockey is on the agenda at the Winter meetings of USA Hockey in January of 2018.

The only team not to jump from the USP3HL to the Elite League appears to be the Alpena Flyers who later folded. Three other organizations are retaining teams in the Elite Division that were already members but not in the USP3HL, the P.A.L. Jr. Islanders, Jersey Hitmen, and Team Beijing. The New Jersey Rockets organization (who joined from the EHL) was adding a team to the Elite Division.

The Bay State Breakers and the River Falls Renegades appear to have withdrawn from the USPHL after having teams in the Elite Division in 2016-17. The Springfield Pics stay in the Premier Division and don't elevate to the NCHC. The Connecticut Jr Rangers return from leave and join the Premier Division. In addition to the six organizations joining from the EHL new organizations include the Spooner, Wisconsin based Wisconsin Muskies; the York, Pennsylvania based Skipjacks Hockey Club, and the Harrington, Delaware based Delaware Lightning. Eight additional organizations made the jump from the USP3HL to the Premier Division (Daytona Racers, Decatur Blaze, Florida Jr. Blades, Ironwood Fighting Yoopers, Kalkaska Rhinos, Motor City Hawks, Traverse City North Stars, and Tri-City Ice Hawks.

On May 18, 2017 the league announced that the Hudson, Wisconsin based Hudson Havoc organization will be added to the Premier Midwest West Division for the 2017-18 season. The team will be based out of the Hudson Sports & Civic Center. The West Sound Warriors announced they would go dormant for the 2017-18 season, but the Vancouver Rangers of the Western States Hockey League relocated to Bremerton, Washington and would assume the Warriors identity in the WSHL. The Seattle Ravens announced they would be renamed the Kent Ravens and join the WSHL as well.

On June 14, 2017 the USPHL announced the addition of the Minnesota Blue Ox who will be based out of the Coon Rapids Ice Center in Coon Rapids, Minnesota.  The team's ownership includes Minnesota Wild head coach Bruce Boudreau, his wife Crystal, and Jay Witta who will be the head coach and general manager.  The team will play in the Premier Midwest West Division.
