Leslie Paul Kozak (born October 28, 1940 in Dauphin, Manitoba) is a Canadian former ice hockey left winger who played 12 games in the National Hockey League (NHL) for the Toronto Maple Leafs during the 1961–62 season.
He played junior hockey for the Melville Millionaires, Saskatoon Quakers, Toronto Marlboros, and the St. Michael's Majors from 1955 to 1960. After missing the 1960-61 season studying in a seminary, he played 12 games for the Toronto Maple Leafs in 1961-62 and scored a goal. He was then sent down to the Rochester Americans where he suffered a career-ending head injury that season.
